The author decribes a cognitive-developmental point of view of eating disorders. According to this model, eating disorders with a comorbidity with personality disorders show a D-disorganized-disoriented attachment pattern in the therapeutic relationship, that may become very unstable and turn into an agonistic system with elements of competition, anger, decrease of metacognitive habilities, acting out and, finally, lead to drop out. The author proposes a model of intervention based on increasing cooperation and empathy and, if necessary, structuring a double-setting with collegues following the same intervention perspective.
