Terapia culturalmente sensible: un acercamiento antropológico

Este trabajo ofrece un modelo a la terapia culturalmente sensible para responder a una diversidad cultural en el cuidado de la salud (mental)2. La terapia culturalmente sensible no puede basarse en datos etnográficos puramente comparativos o en el desarrollo de habilidades técnicas. Esto llevaría a demasiadas teorías étnicamente basadas y la importancia del multiculturalismo sería trivializada. La idea de una terapia sensible a la cultura, por tanto, afecta al potencial para una «crisis de relativismo». En nuestro acercamiento, la cultura ya no es considerada como un concepto estático y materializado, sino como un proceso, una creación contextual y la construcción social de significado. Esto implica una perspectiva inter-subjetiva en la cual los comportamientos, pensamientos y sentimientos del clínico y del paciente se convierten en campo de interacción que opera en niveles múltiples, dentro del cual el paciente y el clínico trabajan para construir un significado (cultural). En esta perspectiva los elementos culturales en los procesos de transferencia y contra-transferencia son muy importantes. También deberíamos aclarar que la salud mental, en modelos para diagnóstico y tratamiento, no deberían sólo buscar las dimensiones físicas, sociales y psicológicas, sino también las culturales.
